CAREU · CARESEEKER SERVICES
Older people’s care and funding application support. A two-page A4 brochure covering our services and referral process: Chinese front, English reverse.
Print on A4 at actual size, double-sided, flipping on the long edge.
LESS BACK-AND-FORTH. A CLEARER HANDOVER.
We bring document preparation, policy explanations and service coordination together so clients, families and referring teams can understand what happens next.
Support Chinese-speaking older people, clarify language and dialect preferences, and arrange interpreting where needed.
Focus on people living alone or without reliable support, helping clarify daily needs, consent and appropriate support networks.
We do not charge for professional referrals or application assistance. Care, assessments and other services may involve costs, explained before arrangements are made.
Dedicated staff review documents, follow up applications and explain what information is needed next.
A single intake process helps clarify priority according to discharge timing and care risks, reducing fragmented communication.
A structured referral captures client, family, medical and functional information together, reducing repeated information gathering.
Start discussing home support before discharge. Safe discharge decisions remain with the treating clinical team.
Translate pathways into practical next steps, connecting CareU or other suitable providers according to client choice and availability.
For urgent needs before funding is in place, review available bridging resources or CareU assistance. Support is subject to individual review and available resources.
ONE REFERRAL. APPROPRIATE PATHWAYS.
We help prepare information, explain requirements, contact relevant organisations and follow up applications. Eligibility varies by age, residence, clinical needs, means and accident circumstances.
Coordinate assessment for older people who may benefit from goal-focused restorative support, including suitable post-discharge circumstances. RCP is assessed support, not emergency funding or a replacement for TCP.
Help document lower to moderate daily support needs, including chronic conditions, mobility and function. A diagnosis alone does not determine a classification.
Help describe personal care, everyday assistance and clinical support needs and prepare information for the relevant assessment.
Coordinate information, family communication and professional recommendations for substantial or complex care needs.
Help pursue suitable AT and HM pathways and coordinate the professional assessments, recommendations or quotes required.
Review applicable dementia support and supplements, the Continence Aids Payment Scheme (CAPS), and enteral feeding support. These are not assumed to fall under SAH.
Help contact appropriate equipment, disability or accident-related schemes. Eligibility and payment remain with the relevant scheme or insurer.
Connect carer and community health support, and clarify existing TCP or CHSP arrangements to reduce gaps between services.
Review safety, needs and available resources for temporary support or individual CareU assistance. Pending government approval does not guarantee bridging funding.
The SAH groupings 1–3, 4–6 and 7–8 above are communication aids, not clinical allocation rules. Formal classifications are determined through aged care assessment. Government schemes may pay for eligible, approved services; contributions, third-party costs or separate approvals may apply.
CHOOSE THE SUPPORT THAT FITS
Care Seeker is CareU’s name for this coordination approach, not a government program. Government-funded care finder services provide important free navigation support through funded organisations, including not-for-profits.
| Area | Government care finder services | CareU Care Seeker |
|---|---|---|
| Main focus | Help eligible older people with complex barriers and limited trusted support understand and access aged care. | Professional referrals combining social work, case management and allied health coordination for discharge, applications and care at home. |
| People and communication | Support according to program eligibility, local coverage and provider arrangements. | Particular attention to Chinese-speaking older people and those living alone or without family support. |
| Coordination | Follow-up is arranged by the local service according to need and capacity. | A dedicated application team receives information together and reviews next steps according to discharge timing and care risks. |
| Connection to care | Help people understand and connect with appropriate services. | Coordinate CareU or other suitable providers according to client choice, needs and availability. |
| Fees and approval | care finder navigation is free; aged care funding is not automatically approved. | Professional referrals and application assistance are free; service costs, eligibility and funding decisions are explained separately. |
Response and processing times vary with local capacity, information quality, assessment and approval arrangements. We aim to reduce repeated communication and provide clear follow-up; we do not guarantee faster processing than every care finder or not-for-profit provider.
The online form includes all sections of the supplied bilingual referral form. Provide known information; optional fields may be left blank.
Referrals enter CareU’s access-controlled staff system for assignment, call notes and progress updates. Health information, Medicare details, signatures and attachments are not displayed publicly.
A client or authorised representative may sign when present. Otherwise the referring professional can confirm consent already obtained and recorded. Submit only authorised information relevant to this referral.
