MDT stands for Multidisciplinary Team — an occupational therapist (OT), an allied health assistant (AHA), a physiotherapist and a registered nurse, all reading the same monitoring data and the same person, and meeting once a month to set the plan.
The model combines 10% care management with a 15% multidisciplinary-team fee. It leaves 75% for direct care and reduces four core support services to $72/hr during weekday standard hours.
Multi · Disciplinary · Team. Not one person carrying the whole case, but four different professions around the same older person, reading the same data, sitting down together every month to set the plan. That is what the 15% team fee buys — not one more support worker, but a team.
Assesses function and safety at home, sets rehab goals and prescribes equipment or modifications, and leads the monthly plan revision.
Delivers the therapists' prescribed exercises in the home — frequently and affordably, so the plan happens on ordinary days.
Targets gait, balance, falls risk and strength loss with a measured program, and quantifies progress month to month.
Medication, wounds, chronic conditions and vital signs — and calls the team together the moment decline shows.
What makes it a team is not that four professions exist on paper — it is that they read the same record: the monitoring devices in the home (falls sensors, activity, sleep, medication prompts, vital signs), every support worker's visit notes, and what the older person and their family actually say. Each profession sees one face of it — the OT function and environment, the physio gait and strength, the nurse vitals and medication, the AHA whether the daily exercises are being done — and once a month it all goes on one table, so the team can say whether this month was better or worse, and what changes next month.
The conclusion goes into the monthly rehabilitation report, shared with the family on WeChat. Any change to the plan is confirmed with the older person first.

25% is one number, but the 15% MDT team fee is a very different amount of money at Level 1 than at Level 8 — and so are the hours the remaining 75% buys. How it is worked out: annual budget × 15% = the MDT team fee; annual budget × 75% = the funding available for care in the home, converted entirely at the MDT core-service rate of $72/hr. The last column is the weekly hours under the 10% model (90% to care, personal care at $100/hr); green is the extra the MDT model buys. Weekday standard hours (06:00–20:00); non-standard hours, weekends and public holidays attract surcharges, two-hour minimum.
