When time becomes precious, this pathway provides a one-off ~$25,000 over 12 weeks so a loved one can spend the final chapter at home — comfortable, free of pain, surrounded by love.
For a diagnosed life-limiting illness with a ~3-month prognosis and the wish to remain at home, with medical evidence. Urgent priority funding; referrals accepted even before joining SAH. AT can be used alongside; home modifications cannot.
Palliative nursing, pain & symptom management, spiritual care (Buddhist or Christian), family guidance and 24/7 phone support — coordinated with your GP and hospital palliative team.
Using the classification and the split you chose above. The 25% MDT model takes a bigger management fee but charges $72/hr for domestic help and personal care, so the honest answer depends entirely on your service mix.
Hours are indicative, at weekday standard rates. The MDT model also includes a monthly multidisciplinary case conference, a written rehabilitation report and CareU paying your government co-contribution up-front — none of which appear in the hours above.
